Health Net approves, ModivCare schedules.

Health Net contracts ModivCare for its Medi-Cal members’ NEMT and NMT, but the authorization comes from Health Net’s own Care Ride Unit. What that means for a provider in ModivCare’s network.

Who does what

Health Net’s provider manual names ModivCare as “Health Net’s contracted provider for all covered non-emergency medical transportation (NEMT) and non-medical transportation (NMT) services” for members assigned to a direct network provider or to shared-risk participating physician groups. Groups and hospitals that carry the NEMT risk themselves must authorize and coordinate with their own transportation provider, so some Health Net members’ rides come from a medical group rather than from ModivCare.

The approval stays with Health Net. Medical offices fax the PCS form to Health Net’s Care Ride Unit to get prior authorization before contacting ModivCare, and Health Net tells ModivCare about approved authorizations every day. The current form (25-183) is valid for up to one year from the provider’s signature; an older version that sends the form to ModivCare is out of date.

Notice and waiting

Health Net’s 2025 tip sheet gives these lead times: urgent services such as dialysis, chemotherapy and discharges up to 4 hours; NMT 24 hours; NEMT 48 hours; mass transit 5 days. ModivCare’s scheduling team works Monday to Friday, 7 a.m. to 7 p.m.

Health Net tells members that drivers are only required to wait 15 minutes past the pickup time, and rideshare drivers 5. That is the number a no-show on a Health Net trip is measured against.

Where Health Net runs Medi-Cal

Health Net lists Amador, Calaveras, Inyo, Los Angeles, Mono, Sacramento, San Joaquin, Stanislaus, Tulare and Tuolumne counties, with footnotes for Los Angeles and Sacramento. It also runs the transportation process for CalViva Health (Fresno, Kings and Madera) and Community Health Plan of Imperial Valley, each with its own PCS form.

Not in the plan’s published documents

We couldn’t confirm these from Health Net’s own pages, so ask the plan or its vendor:

  • How a transportation company joins ModivCare’s network for Health Net members.
  • The claim and billing rules for transportation companies on Health Net trips.

Running Health Net trips in MileCall

ModivCare’s trip file comes in as CSV or Excel with a saved mapping; trips from a medical group that carries its own NEMT risk can be a separate payer with its own rates.

The payer’s minimum wait is part of the no-show screen in the driver app: a timer against the minimum, a photo, a reason and a location check.

MileCall isn’t connected to Health Net or its vendor; trips come in as files you upload.
