ModivCare’s role
Anthem’s California Medicaid provider manual (effective July 1, 2026) says Anthem contracted with ModivCare Solutions to coordinate transportation, and lists what ModivCare is responsible for: taking reservations from members, contracting with ground transportation providers, and providing payment for non-emergency ground transportation claims. Joining the network and getting paid both go through ModivCare.
Notice
Members book at least five business days ahead, and ten for long-distance trips. No advance notice is needed for urgent appointments for dialysis, chemotherapy, radiation therapy, urgent care, wound care or facility discharges: the trips that most often come in at short notice. Members call ModivCare’s line for Anthem, Monday to Friday, 7 a.m. to 7 p.m.
The PCS form
Physicians complete Anthem’s PCS form and return it to ModivCare within two business days of receiving it. The form offers ambulance (including advanced and basic life support), gurney or litter van, wheelchair van and air, and authorizes up to 12 months.

Running Anthem Blue Cross trips in MileCall
ModivCare’s trips come in as a CSV or Excel file with a saved mapping, and the trip sheet goes back in the columns you set up for that payer.
Same-day dialysis and discharge trips can be shopped against every run that could take them, best fit first, before you accept them.
MileCall isn’t connected to Anthem Blue Cross or its vendor; trips come in as files you upload.
